The Health Center is dedicated to meeting the needs of the people we serve. In this section, you will find information on topics that we know are important to you and your family, including health information, community resources and payment information.


Access to Care

Are you uninsured or underinsured? Or maybe you are in need of SNAP (Supplemental Nutrition Assistance Program)? No matter what your need is, our ACCESS to Care team can help. Our team is made up of certified enrollment specialists who will guide you to get the benefits you need. Available options include SNAP (food stamps), Medicaid A, B & D, our Sliding-Fee Discount Program and health insurance through Access Health CT. CDL Physical Exams
CDL Physical Exams

Are you a commercial driver in need of a medical exam? We can help. You can schedule a state Department of Transportation Commercial Driver’s License physical exam with our certified provider at our Ansonia office Mondays through Fridays during normal business hours. Drivers must bring their driver’s license, Department of Transportation (DOT) physical form and the Medical Examiner’s Certificate with them to the visit. Sections one and two of the physical form should be completed prior to the exam. Drivers must also provide a urine sample at the time of the exam.

We recognize that your needs may go beyond the services we offer. That’s why we are providing you with this link to Connecticut 2-1-1. On the Connecticut 2-1-1 website, you will be able to find information on things you may need such as utility assistance, food pantries, housing, legal assistance and much more. If, after visiting the site, you still can’t find what you are looking for, please call 2-1-1 directly for more assistance.
