MyChart, our secure online patient portal, gives you access to all of your Cornell Scott-Hill Health Center health information whenever you need it, wherever you may be. Sign up today.


To sign up for MyChart you need an activation code, which you can get in one of three ways:

  • Find one on your after-visit summaryFind one on your after-visit summary.
  • Request one at your next appointment.
  • Request one online.

Proxy Access


 Need to manage the health information of someone under your care? Need to manage the health information of someone under your care?

Fill out a minor proxy access form (for patients under the age of 18) or an adult proxy access form (for patients ages 18 or older).

Fax the form to 203-688-8155 or email a copy to mychart.eHIM@ynhh.org.

Most requests are handled within 10 business days.
